Technical Assistance:
Discussed the importance of proper diapering procedures and reviewed the required 2 step process.

Suggested relocating the diapering area further away from toys and equipment to ensure the safety of the children playing on the floor.

Comments:
An unannounced Monitoring inspection was conducted today. A total of 6 children (14pts) with 2 staff providing supervision was onsite. The children were preparing for lunch time. They were seated at the table while staff were observed serving lunch. Lunch today was chicken with rice and fresh slices of cucumber. They had completed a morning activity which had the children paint and color the letter "N". The artwork was observed drying on bookshelves. After lunch, the children transitioned to playtime. The younger children were placed in cribs for nap time. The older children transitioned smoothly to their designated cots and quickly fell asleep. Observation of the all programming areas was made. Consultation provided regarding the area used for diapering. No medication needs and no food allergies were identified. A total of 4 children's records and all staff records were reviewed. Description: Based on observation of the family day home, topical ointments were accessible to children.
Evidence:
Topical ointments were accessible in the crib room and also below the diaper changing surface.

Plan of Correction: All topicals were moved to an area not within reach of children.

Standard #: 8VAC20-800-60-B
Description: Based on child record review, a child's record was incomplete.
Evidence:
A second emergency contact was not found for Child #3.
The child's start date was not found.

Plan of Correction: A second emergency contact will be obtained for this child.

Standard #: 8VAC20-800-90-A
Description: Based on child record review, immunizations for one child were not obtained.
Evidence:
Child # 1 began care on 04/25/23, immunizations were not obtained prior to start date and are not on file.

Plan of Correction: The immunizations for this child will be obtained from the parent and will be obtained every six months until the child turns 2 years.

Description: Based on observation of staff diapering two children, hand washing requirements were not followed.
Evidence:
Staff #2 failed to wash their hands after diapering one child and proceeded to diaper another child directly afterwards.

Plan of Correction: Staff was reminded that they must wash hands after every diaper change

Standard #: 8VAC20-800-680-B
Description: Based on observation of staff diapering 2 children, children's hands were not washed.
Evidence:
Staff #2 failed to ensure that Child # 4 and Child #3's hands were not washed after diapering.

Plan of Correction: Staff was reminded that all children's hands will be washed after being diapered.

Standard #: 8VAC20-800-690-D
Description: Based on observation of staff diapering, the diaper surface was not cleaned and sanitized.
Evidence:
Staff #2 did not clean with soap and water and sanitize with bleach and water after diapering two children.

Plan of Correction: Two spray bottles will be made, one for soap and water and the second for bleach and water. Staff will be required to clean and sanitize the diaper surface after every diaper change.
