Technical Assistance:
Provided consultation on standard:
1) 60B: All information is required on the child registration form. Encourage parents to use "NA" rather than leaving a space blank if the information doesn't apply. The DOE model form found on our website, when complete, will have all required information. Written authorization is required if the child is to be taken off of the premises of the family day home.
2) 200A: the provider shall orient the assistant by the end of their first week of assuming job responsibilities
3) 250B: children shall be protected from materials that could be swallowed or present a choking hazard. A pacifier attached to a ribbon with small beads is a choking hazard. Sleeping with a bib is a potential choking hazard.
4) 340C: best practice is that the bathroom contain toilet paper and paper towels within reach of children to develop independence
5) 680B: Caregivers shall ensure that children's hands are washed with liquid soap and warm running water after diapering.
6) 690D: the diapering surface shall be cleaned and sanitized after each use. (sanitized means treated in such a way as to remove bacteria and viruses from inanimate surfaces through first cleaning and secondly using a solution of bleach water which is sprayed on and then allowed to air dry.)
7) 760A: the first aid kit shall include triangular bandages (indicating more than one) and a first aid manual.
8) 870: the family day home shall schedule meals and snacks so there is a period of no more than three hours between the meals and snacks.

Comments:
An announced initial inspection was conducted from 10:00 am - 12:00 pm with the provider and her assistant. There were three children in care, ranging from five months to two years of age, for a total of eight points. Children were observed playing with age-appropriate toys and the infant was observed sleeping, waking, having a diaper change and playing. Three child records and three caregiver/household member records were reviewed. The childcare area was observed and is large, organized and well equipped with age-appropriate toys, equipment and materials. One crib that meets CPSC standards (manufactured 4/17) was observed. Reviewed and discussed the written attendance, first aid kit, battery operated flashlight and weather band radio, emergency preparedness plan, weekly menu, required posted information, and child and caregiver records. The provider has current certification in CPR and First Aid, but is not Medication Administration Training (MAT) certified and cannot administer medication to children in care. Standard #: 8VAC20-800-940-B
Description: Based on observation and interview, the home did not have an operable thermometer available to monitor the refrigerator and freezer temperatures. Evidence: the provider stated that she did not have a thermometer.

Plan of Correction: Provider stated she will purchase a thermometer for the refrigerator and freezer.

Standard #: 8VAC20-800-60-B
Description: Based on review of three child records, and an interview with the provider, not all records contained all required information. Evidence: The record for child #1, child #2, and child #3 did not contain complete emergency contact information, first date of attendance, parent's signed acknowledgement of the information required in standard 70, proof of the child's age and identity, results of the health examination, or written authorization for emergency medical care.

Plan of Correction: Provider stated she will obtain the missing information.

Standard #: 8VAC20-800-70-A
Description: Based on review of three child records, and an interview with the provider, not all parents were provided, in writing, all required information for parents before the child's first day of attendance. Evidence: The record for child #1, child #2, and child #3 did not contain documentation of receipt of all required written information including information for parents, policies on the administration of medication, whether or not there is liability insurance, and provisions of the family day home's emergency preparedness and response plan.

Plan of Correction: Provider stated she will provider parents with the required information.

Standard #: 8VAC20-800-90-A
Description: Based on review of three child records, the provider did not obtain documentation that each child has received immunizations prior to the child's first attendance. Evidence: the record for child #1 (date of first attendance: 11/21/2023), child #2 (date of first attendance: 7/28/2021) and child #3 (date of first attendance: 5/8/2023) did not have documentation of immunizations.

Plan of Correction: Provider stated she will ask parents to provide the immunization records.
