Comments:
A monitoring inspection was initiated and concluded on 8/29/2023. There were 2 daycare children and one household child present with the licensee supervising.The point total was 6. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 caregiver / Household member records were reviewed. The inspection started at 2:20 pm and concluded at 4:10 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.036-C
Description: Based on a review of records and interview, the licensee did not undergo a background check prior to employment and every 5 years thereafter.
Evidence:
The record for caregiver #1 contains the most recent documentation of results of a central registry search which is dated 6/29/2018 - more than 5 years ago.
The licensee was unable to locate documentation that a more recent search of the central registry had been conducted.

Plan of Correction: The licensee will request an updated Cent4ral Registry report for caregiver #1 within 10 days and send a copy of the request to the inspector. The results will be forwarded to the inspector and filed in the caregiver's record upon receipt.

Standard #: 8VAC20-800-780-B
Description: Based on a review of records and interview, the licensee was unable demonstrate that each child's record is reviewed annually with the child's to ensure that the emergency contact information required in 8VAC20 800 60 B 2 to ensure the information is correct with the parent's signed acknowledgment of the review.

Evidence:
The records for child #3 (enrolled 11/30/2018) and child #4 (enrolled 1/11/2022) did not have documentation that the records had been reviewed by the parents and updated for any changes to the emergency information, as applicable, annually.

Plan of Correction: The licensee will request that the parent of child #3 and child #4 review and update their children's records for any changes to the emergency information, and sign that the record was reviewed within 10 days and, again, annually.

Standard #: 8VAC20-800-60-B
Description: Based on a review of records and interview, the licensee was unable demonstrate that each enrolled child's record contains all of the required information and documentation.
Evidence:
The record for child #1 who enrolled for care on 2/21/2023 did not contain the policy number for the parent's hospitalization plan.

Plan of Correction: The licensee will request this information form the parent today and ask the parent to document in writing by the end of the week if she is unable or unwilling to provide this information.
