Technical Assistance:
Discussed the Out of State Background Check requirements and the need to do the Child Abuse and Neglect Check for Florida by 9/22/2024 for one of the Caregivers.

Comments:
An unannounced monitoring inspection was conducted today from 9:38am-10:30am with the provider on site. There were 11 children with 3 caregivers. A sample of 2 children's files and 5 caregivers/household member's files were reviewed. The children were engaged in the following: playing outside. Interactions between the children and caregivers were positive. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. The physical space, attendance records and drill record were reviewed. There were no medications on site. Areas of non-compliance are identified in the violation notice.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use caregiver names; list caregivers by positions only.

Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Standard #: 22.1-289.035-B-4
Description: Based on review of caregiver records, the family day home did not obtain the results of the Sex Offender Registry for each state an individual has resided in the past 5 years prior to the 1st day of employment.
Evidence:
1. Caregiver #3 (Hire date 8/22/2023) lived in another State (Florida) in the last 5 years and had the sex offender registry results in the file dated 9/14/2023.

Plan of Correction: I will make sure to do it prior to hire in the future.

Standard #: 8VAC20-770-40-D-4
Description: Based on review of three household members records, the provider did not obtain a completed sworn disclosure statement for each household member within 30 days of their becoming 18 years old.
Evidence:
1. Household Member #3 did not have a sworn disclosure statement on file and had been over 18 years old for more than 30 days.

Plan of Correction: I will have them fill out today.

Standard #: 8VAC20-800-170-A
Description: Based on documentation review, the provider did not obtain from each caregiver at the time of hire and each adult household member prior to coming into contact with children a current report of Tuberculosis Screening form documenting the absence of tuberculosis.
Evidence:
1. Caregiver #3 (Date of hire 8/22/2023) did not have a Tuberculosis Screening form on file.
2. Household Member #3 did not have a Tuberculosis Screening form on file and has been an adult for more than 30 days.

Plan of Correction: I will get an appointment for both of them.
