Comments:
A renewal inspection was conducted on 09/18/2023. There were a total of 2 children (4 points) observed with 2 caregivers present. The children were engaged in the following: toy play and napping. The inspector reviewed compliance in the areas of administration, personnel and household members, physical environment, care of children, handwashing, diapering, medication, emergencies and nutrition. A total of 2 child records and 4 caregiver/household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 22.1-289.036-A
Description: Based on a review of records, the provider did not ensure finger prints were repeated every 5 years.
Evidence:
Caregiver #1 had finger prints that expired on 3/5/2023.
Household member #1 had finger prints that expired on 8/3/2023.
Household member #2 had finger prints that expired on 8/2/2023.

Plan of Correction: I have scheduled the appointments.

Standard #: 22.1-289.058
Description: Based on observation, the provider did not ensure there was at least one carbon monoxide detector in the family day home.
Evidence:
There was no carbon monoxide detector available in the family day home.

Plan of Correction: I will order a carbon monoxide detector.

Standard #: 8VAC20-800-80-A
Description: Based on a review of child records, the provider did not ensure required information.
Evidence:
Child #1 (start date 8/28/2023) did not have documentation of age and identity in their record.

Plan of Correction: I am going to ask the mom to bring the birth certificate.

Standard #: 8VAC20-800-90-A
Description: Based on a review of child records, the provider did not ensure they had documentation of immunizations prior to attending the family day home.
Evidence:
Child #1 (start date 8/28/2023) did not have immunizations in their record.

Plan of Correction: I am going to ask the mom to bring them.

Standard #: 8VAC20-800-180-A
Description: Based on observation, the provider did not obtain for each caregiver a current Report of Tuberculosis every two years from the date of the first screening.
Evidence:
Caregiver #2 had a TB test that expired on 8/19/2023.

Plan of Correction: We will schedule a tb test.

Standard #: 8VAC20-800-320-B
Description: Based on observation, the provider did not ensure child-resistant protective covers were installed on all unused electrical outlets accessible to children.
Evidence:
There were 2 uncovered electrical outlets by the toddler bed closest to the entry door.
There was 1 uncovered electrical outlet by the Tv by the entry door.
There was 1 uncovered electrical outlet by the bathroom.
There was 1 uncovered electrical outlet in the baby room.

Plan of Correction: Fixed during the inspection.
