Comments:
A renewal inspection was conducted on 9/7/2023. The inspector arrived at 11:25 AM and departed at 12:50 PM .There were 3 children present, ranging in ages from infant to 3 yrs, with a substitute staff supervising. The provider arrived mid way through the inspection. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 4 staff/household records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the provider during the exit interview.

Violations:
Standard #: 8VAC20-800-60-B
Description: Based upon review of three children's records, the provider has not ensured that the children's records include all required information.
Evidence:
1. The record provided for child 1 did not include the street address of the person to be contacted in an emergency if a parent cannot be reached.
2. The parent of child 2 did not sign the Provisions of the Emergency Preparedness Plan form indicating that the parent was aware of the plan.

Plan of Correction: The provider responded with the following:
All children's records will be reviewed to ensure that all required information is included in the child's record.

Standard #: 8VAC20-800-240-A
Description: Based upon observation, the provider has not ensured that areas of the family day home, inside and outside, are maintained in a safe condition.
Evidence:
There is a piece of plywood near the back door from the porch that is covering an open area under the house. The plywood is not securely attached on all sides, creating a potential pinch point between the plywood and the wall.

Plan of Correction: The provider responded with the following:
The board will be securely attached to the wall to ensure that children's fingers cannot be pinched.

Standard #: 8VAC20-800-250-B
Description: Based upon observation, the provider has not ensured that there are not materials that could be swallowed or present a choking hazard accessible to the children.
Evidence:
There were three children in care three years of age and younger. There was a 1 inch plastic tube from a marker, 1/2 inch by 1/2 inch connector cubes and a small counting bear on the floor of the activity area. These items were potential swallowing or choking hazards.

Plan of Correction: The provider responded with the following:
Routine sweeps will be made to ensure there are no small pieces of equipment accessible to the children. Those learning supplies that are of a choking size will only be used with direct caregiver supervision at all times.
