63.2 CHILD ABUSE AND NEGLECT

An unannounced focused monitoring inspection was conducted on 09/19/2023. There were 9 children, ages 13 months-3 years, totaling 20 points, one substitute caregiver and one assistant caregiver present during the inspection. The inspector reviewed compliance in the areas of personnel, physical equipment and environment, and care of children. The children were observed during rest time. The inspector arrived for the inspection at 1:50 pm and departed at 2:30 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #: 8VAC20-800-140-D Description: REPEAT VIOLATION
Based on document review, the provider failed to ensure that a substitute provider shall record and sign the time of arrivals and departures on each day that the substitute provider works.
Evidence: The substitute provider log was not completed by Caregiver 1 on 09/18/2023 (the departure time was not listed) or 09/19/2023 (the arrival time was not listed). Caregiver 1 was in the role of the substitute provider on those dates.Plan of Correction: Will sign the document each time.
Standard #: 8VAC20-800-320-B Description: REPEAT VIOLATION
Based on observation, the provider failed to ensure that child-resistant covers shall be installed on all unused electrical outlets and surge protectors accessible to children under five years of age.
Evidence: There was an uncovered electrical outlet on both sides the couch that were accessible to children under five years of age.Plan of Correction: The outlet was covered during the inspection. The outlets will be covered after the provider vacuums.
