Comments:
A non-mandated monitoring inspection was conducted on September 20, 2023 from 10:10 A.M.-12:30 P.M. There were four children present for a total of 15 points, ranging in ages from eight months to two years of age, with one caregiver and one volunteer supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and one caregiver record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-60-B
Description: Based on a review of children records, the provider failed to ensure all required information was in the children records.

Evidence:
1. Child #1's record did not contain the work phone number for Parent # 1.
2. Child #2's record did not contain the work phone number for Parent #1 and #2, and did not contain names of persons who are authorized to pick up the child.
3. Child #3's file did not contain the work phone number for Parent #1 and Parent #2.
4. The provider verified that the records were missing information.

Plan of Correction: Information received from parents at pick-up. In the future review all paperwork to ensure all items are completed.

Standard #: 8VAC20-800-70-B
Description: Based on a review of children records, the provider failed to obtain the parent's written acknowledgment of the receipt of the written information required.

Evidence:
1. Child #1's record did not contain written acknowledgement of the receipt of the provisions of the family day homes emergency preparedness and response plan.
2. Child #2 and #3's record did not contain written acknowledgment of the receipt of the policies for medication administration, and whether or not the provider has liability insurance in force on the family day home operation.

Plan of Correction: Forms completed at pick up and added to bright wheel.

Standard #: 8VAC20-800-680-A
Description: Based on observation, the provider failed to ensure that all caregivers shall wash their hands with liquid soap and running water before feeding or helping children with feeding.

Evidence:
Volunteer #1 was assisting at lunchtime. The provider handed Volunteer #1 the food to feed to Child #4 for lunch. Volunteer #1 was sitting on the floor in front of the child in the highchair.
Volunteer #1 did not wash their hands with soap and warm running water before she began to feedChild #1.

Plan of Correction: Review licensing standards with volunteer and have them complete the health and safety training.

Standard #: 8VAC20-800-680-B
Description: Based on observation, the caregiver failed to ensure that children's hands were washed with liquid soap and warm running water before eating.

Evidence:
1. Child #4's hands were not washed before eating lunch.
2. The provider verified that the child's hands were not washed before eating lunch.

Plan of Correction: Review handwashing requirements with volunteer and have them complete the health and safety training.
