Comments:
An unannounced renewal inspection was conducted on September 27, 2023, from 10:15am-11:20am. At the time of entrance, ( 12 ) children (including a live in grandchild) ( 30) points were in care with ( 3) caregivers present. The inspector reviewed 5 children?s records and 3 caregiver/household member records. The 3 level home and cribs were inspected this day. Due to the weather, the outside was not inspected. Children were observed waking from naptime, coloring, legos, bathroom breaks and hand washing. No medications were on site. Areas of non-compliance are identified on the violation notice.

Violations:
Standard #: 22.1-289.058
Description: Based on observation and provider interview, the home is not equipped with a carbon monoxide detector.
Evidence: There was no carbon monoxide detector in the home, the provider was not aware that she needed one.

Plan of Correction: The provider will purchase and install a carbon monoxide detector.

Standard #: 8VAC20-800-760-A
Description: Based on a review of the first aid supplies, there is not triangular bandages maintained in the firs aid kit.
Evidence: There were no triangular bandages in the first aid kit.

Plan of Correction: The provider will purchase triangular bandages and place them in the first aid kit.

Standard #: 8VAC20-800-100-A
Description: Based on a review of children records, the provider has not obtained documentation of each child's physical examination within 30 days after the first day of attendance.
Evidence: Child #1, first day of attendance 1/3/2023 and child # 3, first day of attendance 2/15/2023 does not have documentation of a physical examination.

Plan of Correction: The provider requested the physicals from the parents today.

Standard #: 8VAC20-800-70-A
Description: Based on children record review, parents have not been provided the policies of administration of medication for each child before the child's first day of attendance.
Evidence: Child # 4, date of first attendance 8/14/2023 record does not have documentation of the policy for administration of medications.

Plan of Correction: The provider contacted the parent for this information today.

Standard #: 8VAC20-800-180-A
Description: Based on a review of caregiver records, there is not documentation that a a current TB screening has been obtained.
Evidence: Caregiver # 3 does not have documentation of a current TB screening in the file.

Plan of Correction: The caregiver will find the TB results and file in the caregivers record.
