Comments:
An unannounced monitoring inspection was conducted on September 27, 2023 between the hours of 9:45 am through 11:30 am. There were a total of two children (7 points) observed with two caregivers. A total of two children's files and three staff/household member's files were reviewed. The children were engaged in the following: toy play, games, television time, snack, hand washing, outdoor time and other organized activities. Violations:
Standard #: 22.1-289.036-A
Description: Based on record review, the family day home did not update criminal history record reports every five years.
Evidence:
1. Criminal history record reports have to be updated every five years. Staff #1's record contained an expired criminal history record report. Documentation of the criminal history record report was dated 08/23/2018.
2.Criminal history record reports have to be updated every five years. Household Member #1's record contained an expired criminal history record report. Documentation of the criminal history record report was dated 08/22/2018.
3.Criminal history record reports have to be updated every five years. Household Member #2's record contained an expired criminal history record report. Documentation of the criminal history record report was dated 08/29/2018.

Plan of Correction: Appointments with Fieldprint have been made to get fingerprints taken for the provider and household members.

Standard #: 8VAC20-800-960-E
Description: Based on observation, bottles were not labeled with the child's full name and date when more than one infant was in care.
Evidence:
1. Child #1's bottle(Start date-07/25/2023) was located sitting on top of a shelf without the child's name and date.
2. Child #2's bottle (Start date-07/31/2023) was not labeled with the child's name and date.

Plan of Correction: Bottles will be labeled with the child's full name and date daily.

Standard #: 8VAC20-800-690-D
Description: Based on observation, the diapering surface was not cleaned and sanitized after each use.
Evidence:
1. Staff #1 was observed changing a child's diaper. After the diaper change, Staff #1 wiped the diapering surface with a baby wipe and did not clean and sanitize the diaper pad.

Plan of Correction: The provider will use a cleaning and sanitizing solution to clean and sanitize the diapering surface after each use.
