Comments:

An unannounced monitoring inspection was conducted on September 27, 2023 from 2:15 PM- 3:45 PM. At the time of entrance, 5 children (age range 7 months to 2 years old) totaling 17 points were in care with 2 providers. The sample size consisted of 5 children?s records, 2 provider records and 2 household member records. Children were observed participating in their rest period and preparing for their afternoon routine. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program

Description:	Based on record review and provider interviews, the licensee did not ensure to obtain updated background checks as required every 5 years. Evidence: Provider #1?s record did not include an updated Criminal Records Check (CRC). The most recent CRC is dated 8-29--2018. Provider #1 confirmed they had not yet submitted the CRC.

Description:	Based on record review and provider interviews, the provider did not ensure that a person living in the home of a licensed family day home provider had a sworn statement or affirmation when the person age 18 years or older begins residing in the home. A search of central registry (CPS) and criminal history record check (CRC), as requested by the individual must be done within 30 days of a person 18 years of age or older beginning to reside in the home. Evidence: Household member #4 began residing in the home on 4-13-2023 and they did not have a CPS check or CRC .
