Comments:
A monitoring inspection was initiated on 9/18/2023 at approximately 8:50am and concluded on 9/18/2023 at approximately 9:50am. There were no children in care during the inspection. The inspector reviewed compliance in the areas of administration, personnel and household members, physical environment, care of children, handwashing, diapering, medication, emergencies and nutrition. A total of 5 child records and 5 caregiver/household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 22.1-289.036-B-2
Description: Based on review of household residents information, the provider did not obtain updated criminal history checks for themselves and household members.

Evidence: Criminal history records checks were expired for the provider and household member #3 and household member #5.

Plan of Correction: Provider will obtain updated criminal history record checks for themselves and household members.

Standard #: 8VAC20-800-90-A
Description: Based on review of children's records, the provider did not obtain documentation that the child had been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations before the child attended the family day home.

Evidence: Immunization records were not available to review for Child #1.

Plan of Correction: Provider will obtain immunization records for the child.
