Technical Assistance:
Discussion that the fingerprint checks on file for Staff #1 and Household Member #1 are set to expire later this month. These background checks are only good for 5 years from the date of the finding (last finding was from October 2018). In addition, the CPS check for Household Member #1 (also dated October 2018) is set to expire later this month as well.

Comments:
This monitoring inspection was initiated by licensing staff and concluded on 10/05/2023, between the hours of 11 am/pm to 12:30 pm. There were 5 children present (17 points), ranging from infant to preschool-age, with 3 caregivers supervising. The children were engaged in the following: games, bathroom breaks, hand washing, diapering, song, music, outdoor play, lunch, and other organized activities. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 5 children records, 3 staff records, and 2 household records were reviewed.

Information gathered during the inspection determined that there were (3) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the home.

Violations:
Standard #: 8VAC20-800-780-B
Description: Based on review, 2 of 5 children files indicated that the child's parent did not annually provide a review of the parent's emergency contact information to ensure that the information is correct.

Evidence -
1. On the date of inspection (10/05/2023), the last documented review of the emergency contact information for Child #1 was dated - 05/05/2022.
2. The last documented review of the emergency contact information for Child #4 was dated - 04/28/2021.

Plan of Correction: This information will be reviewed, (updated if needed), and signed as verification.

Standard #: 8VAC20-800-90-A
Description: Based on review, 2 of 5 children files indicated that the Provider did not obtain documentation that the child has been adequately immunized (according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations).

Evidence -
1. On the date of inspection (10/05/2023), the following children did not have documentation that each had been adequately immunized within their files: Child #2 (start date - 09/05/2023), and Child #3 (start date - 09/25/2023).
2. While an immunization page was missing entirely for Child #2, on the health forms for Child #3 their immunization page had a note from their doctor's office saying, "see attached." However, no other immunization documentation was attached to this form.

Plan of Correction: An immunization page will be obtained and placed on file.

Standard #: 8VAC20-800-90-C
Description: Based on review, 2 of 5 children files indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations (for children who were not exempt from the immunization requirements according to subsection B of this section) once every six months for any child in care who is under the age of two years.

Evidence -
1. On the date of inspection (10/05/2023) the following children did not have an updated immunization page available for review: Child #1 (last on file - 05/05/2022), and Child #5 (last on file - 03/07/2023).
2. On the date of inspection, both Child #1, and Child #5 were under the age of two years.

Plan of Correction: An updated immunization page will be obtained and placed on file.
