Evidence: Household member 1 does not have an updated fingerprint based criminal history record check. It was last conducted on 8/16/2018. Household member 1 also does not have an updated sworn disclosure statement. It was last updated 4/25/2018.Plan of Correction: The background check was completed on 10/4/2023.
Standard #: 8VAC20-800-920 Description: Based on record review, the family day home did not ensure menu was dated.
Evidence: The menu from the week of 9/25/2023 was not dated.Plan of Correction: We will take out the trampoline.
Standard #: 8VAC20-800-210-B Description: Based on record review, all staff have not completed 16 hours of required annual training.
Evidence: Staff 2 has 10 of 16 required hours of training in 2022.Plan of Correction: We will make sure to get the training hours in 2023.
Standard #: 8VAC20-800-500-F Description: Based on observation, trampolines were in use during the hours children are in care.
Evidence: There is a small indoor child's trampoline in the corner of the child care area and children were observed jumping and climbing on it.Plan of Correction: We will remove the trampoline.
