Comments:
An unannounced renewal inspection was conducted on October 10, 2023, from 9:45am-11:00am. At the time of the inspection, 5 children (17pts) were in care with three caregivers present. The inspector reviewed six children's records and four caregiver's records and one household member record. Children were observed during handwashing, diapering. television time, singing, and playing with toys. Areas of non-compliance are identified on the violation notice. Repeat violations were found and are identified within this report.

Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return it to the department within five business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only.

Any plans of correction not received within five business days will result in the plans not appearing on the Violation Notice and public website.

Violations:
Standard #: 22.1-289.035-B-2
Description: Based on record review, the family day home did not obtain a criminal history records check prior to the first day of employment.
Evidence:
1. Caregiver #4 was hired on May 8, 2023. Caregiver #4's record contained criminal history records check results dated 05/30/2023.

Plan of Correction: Moving forward when staff are hired, fingerprint results will be obtained prior to the first day of employment.

Standard #: 8VAC20-800-800-B
Description: Based on documentation review and interview, the provider did not review the emergency plan at least annually and update the plan as needed.
Evidence:
1. The emergency plan was not updated annually. Documentation of the last annual review was dated 09/19/2022.

Plan of Correction: The provider will review the emergency plan and sign and date for the annual review.

Standard #: 8VAC20-800-200-C
Description: Based on record review and interview, documentation of orientation was not signed and dated by the provider and assistant.
Evidence:
1. Caregiver #2's record (Date of hire-05/08/2022) and Caregiver #4's record (Date of hire-05/08/2023) did not contain documentation or orientation.
2. Caregiver #1 confirmed that Caregiver #2 and Caregiver #4 received orientation, but documentation was not maintained.

Plan of Correction: The provider will print the department's model orientation paperwork from the website and have the assistants to sign and date. Once completed, the provider will document the assistant's records.

Standard #: 8VAC20-800-330-A
Description: Repeat Violation

Based on observation and interview, a landline telephone was not available, operable, and accessible during the family day home's hours of operation.
Evidence:
1. The family day home did not have a landline telephone.
2. Caregiver #1 confirmed that the family day home did not have a landline telephone and that a cell phone was used for communication.

Plan of Correction: The provider will call the local phone company to set up a landline phone. Once the landline is established, the provider will provide proof to the licensing inspector that the landline has been set up.
