Comments:

A monitoring inspection was initiated and concluded on 10/10/2023. There were 5 children present with the provider supervising. The point total was 7. The children were observed during lunch time and nap time. A total of 3 child records, 1 staff record, and 1 family member record were reviewed. The inspection started at 12:55 pm and concluded at 2:40 pm. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #:	22.1-289.011-F
Description:	(Repeat Violation) Based on observation and staff interview, the licensee did not ensure the results of the most recent inspection required by the Superintendent were posted in a conspicuous place on the licensed premises. Evidence: The provider confirmed that the inspection results posted were dated 4/18/23 but the most recent inspection was 6/14/23 and was not posted.
Plan of Correction:	Will post the most recent inspection by 10/11/2023.
Standard #:	8VAC20-800-920
Description:	(Repeat Violation) Based on observation and staff interview the licensee did not ensure when meals and snacks are provided by the family day home, the menu for the current one-week period shall be dated. Evidence: The provider confirmed that the menu that was posted was not for the current week. The menu was dated 9/25-29/2023.
Plan of Correction:	Will post the current week menu by 10/11/2023.
Standard #:	8VAC20-800-60-B
Description:	Based on record review and staff interview the licensee did not ensure a child's record shall contain all of the required information. Evidence: The licensee confirmed that the following items were missing from children's records: 1. The record for child #1 did not contain a 2nd emergency contact. 2. The record for child #2 did not include the work address and phone number for one of the parents listed; immunization records; results of the health examination; and proof of the child's identity. 3. The record for child #3 did not include the home address and phone number for the 1st parent listed; the work name, address, and phone number for the 1st parent listed; and the work phone number for the 2nd parent listed.

Description:	Based on observation the licensee did not ensure the hot water at taps available to children shall be maintained within a range of 105 F to 120 F. Evidence: The licensing inspector using a temperature gauge confirmed that the hot water registered 124.5 F at the sink in the bathroom where the children wash their hands.
