Technical Assistance:
Provided consultation on standard:
1) 320A: small electrical appliances such as, but not limited to, toasters, blenders, can openers, coffee pots/Keurig?s, and irons shall be unplugged unless being used.
2) 500C: Stationary outdoor playground equipment shall be placed at least six feet from other play structures or obstacles.
3) 940C: The provider shall have an operable thermometer available to monitor refrigerator and freezer compartment temperatures.

Comments:
An unannounced renewal inspection was conducted from 10:50am ? 12:20pm with the provider and her two assistants. There were 11 children in care, ranging from one year to four years of age, for a total of 28 points. Children were observed playing with a wide variety of age-appropriate toys, and going on a field trip to the neighborhood playground. Three child records and seven provider/household member records were reviewed along with the daily attendance, emergency drill log, menu and required posted information. The provider has certification in Medication Administration Training (MAT) but stated that there were no medications on site.

Standard #: 8VAC20-800-830-C
Description: Based on review of documentation, and an interview with the provider, it was determined that the emergency evacuation procedures are not documented as required. Evidence: there was no documentation of evacuation drills in January, June or August of 2023.

Standard #: 8VAC20-800-50-A
Description: (Repeat Violation) Based on review of documentation, it was determined that the family day home did not ensure all children were accurately included on the attendance report. Evidence: There were six children listed on the attendance record for 10/11/23. The provider added the other five children to the list after the LI arrived.

Standard #: 8VAC20-800-530-B
Description: Based on observation and interview, not all cribs being used for infants had a tight-fitting bottom cover. Evidence: five out of five cribs had loose sheets. The provider stated that three of the cribs are being used by infants (age 12 months, age 13 months and age 15 months). The LI observed two infants in these cribs. Loose crib sheets are a suffocation hazard and increase the risk of sudden unexplained infant death.
