Comments:

An unannounced renewal inspection was conducted on October 3, 2023 from approximately 10:40 am to 11:45 am. The inspector reviewed compliance in the areas of administration, personnel, household members, physical environment and equipment, emergencies, caregiver training, preventing the spread of disease and nutrition. The provider record and 4 children?s records were reviewed. All areas of the home were observed and areas where the children are cared for were inspected. There were 4 children in attendance for a total of 11 points. The children were observed playing in the playroom while the inspector was on the premises. The first aid kit was inspected and found complete. The menu, emergency numbers, and daily schedule were posted. The information gathered during the inspection determined violations with applicable standards or law. Violations were issued. Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice, sign, and return it to me within 5 business days from today, 10/13/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Description:	Based on documentation review, and interview, the provider failed to document emergency evacuation drills. Evidence: The emergency evacuation drill log did not have documentation of drills for the months of July, August, or September 2023. The provider stated that the drills were conducted but not documented.

Description:	Based on documentation review, and interview, the provider failed to maintain a written record of attendance each day for children in care. Evidence: Child #3 was in attendance. The written record of attendance did not have child #3 written down. The provider confirmed that child #3 was not written on the attendance sheet for that day.
