Technical Assistance:
The following areas of FDH standards were reviewed with the provider:
*Annual training requirements;
*Requirements for emergency practice drills;
*Water temperature shall not exceed 120 F;
*Attendance documentation to occur as child arrives/departs.

Comments:
An unannounced renewal inspection was conducted on 10/13/2023 from 10:30 am - 12:30 pm. At the time of entrance there were five children in care with the provider. The ages of the children counted for a total of 11 points.
Children were observed during morning program time which included large group, block/free play and provider directed table learning activities. Toileting and hand washing was reviewed. Records reviewed for the five children in care in addition to the provider, assistant and one household member.

Violations were found in the areas of administration, equipment and background checks. These violations are listed on the violation notice issued to the family day home and were reviewed with the provider at the conclusion of the inspection.

Violations:
Standard #: 22.1-289.011-F
Description: Based on observation, the provider failed to ensure that the most recent inspection of the FDH was posted in a conspicuous place.

Evidence:
The findings of the 08/19/2021 monitoring inspection were posted however, the most recent monitoring inspection was conducted on 12/08/2022.

Plan of Correction: The provider stated that the current inspection reports will be posted going forward.

Standard #: 22.1-289.035-A
Description: Based on record review and interview, the provider failed to ensure that a completed sworn statement or affirmation shall be completed every five years.

Evidence:
1. The most recent sworn statement or affirmation for caregiver 1 was completed on 01/12/2018.
2. The most recent sworn statement or affirmation for household member 1 was completed on 01/10/2018.
3. The provider confirmed that an updated sworn statement had not been completed for these two individuals.

Plan of Correction: The provider stated that an updated SDS would be completed by caregiver 1 and household member 1.
Updated sworn statements for both individuals were sent to the inspector on 10/13/2023.

Standard #: 8VAC20-800-90-C
Description: Based on record review, the provider failed to obtain documentation of additional immunizations once every six months for children under the age of two years.

Evidence:
1. The most recent immunizations on file for child 1 were administered 04/07/2021.
a. Child 1 is now over the age of two years however, the updated immunizations were not obtained as required.

Plan of Correction: The provider stated that the parent of child 1 will be asked to provide a copy of the current/updated immunization record.

Standard #: 8VAC20-800-500-C
Description: Based on observation, the provider failed to ensure that stationary outdoor playground equipment shall be placed at least six feet from the perimeter of other play structures or obstacles.

Evidence:
1. The outdoor play structure with swings has been placed approximately 2 1/2 feet from a play house and play structure with slide.
2. The outdoor play structure with slide has been placed approximately 3 1/2 feet from the play house.

Plan of Correction: The provider stated that the outdoor play structures would be re-positioned to ensure each are at least six feet apart.
