Comments:

An unannounced monitoring inspection was conducted on October 18, 2023 from 2:53 PM--4:05 PM. At the time of entrance, 2 children (age range 2-3) totaling 4 points were in care with 1 provider. The sample size consisted of 4 children?s records and 2 provider records. Children were observed preparing for their afternoon routine to include snack. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program

Violations:

Standard #:	22.1-289.035-B-2
Description:	Based on record review and provider interviews, the licensee did not ensure to obtain a Criminal History Records Check Prior to 1st Day of Employment. Evidence: Provider #2?s record did not include a Criminal Records Check (CRC) and first date of employment was 10-18-2023. Provider #1 confirmed they had not yet submitted the CRC.
Plan of Correction:	Provider #2 (CRC) has been done, waiting on results, provider #2 will not work until (CRC) is completed.
Standard #:	8VAC20-770-60-B
Description:	Based on record review and provider interviews, the licensee did not ensure to obtain a completed Sworn Statement completed prior to 1st day of employment. Evidence: Provider #2?s record did not include a Sworn Statement and the first day of employment was 10-18-2023.
Plan of Correction:	Child's paperwork has been completed. Policy # is parents SSN-parent works from home same phone number.
Standard #:	8VAC20-800-830-A
Description:	Based on document review and provider interviews, the provider did not ensue the emergency evacuation procedures shall be practiced monthly with all caregivers and children in care during all shifts that children are in care. Evidence: Provider #1 confirmed they did not conduct a practice drill in the month of September 2023.
Plan of Correction:	A practice drill will be conducted each month going forward. For extra practice a 2nd drill was done.
Standard #:	8VAC20-800-990-B
Description:	Based on observation and provider interviews, the provider did not ensue that during transportation of children the driver has met all the requirements. Evidence: Provider #1 transported 2 children for afterschool pick up on 10-18-2023 and they confirmed they did not have emergency supplies as required (first aid kit).
Plan of Correction:	A second first aid kit has been purchased.
Standard #:	8VAC20-800-120-A
Description:	Based on record review and provider interviews, the licensee did not ensure to maintain a record for each caregiver. Evidence: Provider #1 verified there was not a record available for review for Provider #2 (first date of employment 10-18-23).
Plan of Correction:	Provider #2 records are being completed. Provider #2 will not work until completed.
Standard #:	8VAC20-800-60-B
Description:	Based on record review and provider interviews, the provider did not ensue to maintain an up-to-date record at the family day home for each enrolled child with all the required information. Evidence: Provider #1 confirmed the record for Child #4 did not include the insurance policy number of the child?s medical insurance and one of the parents employment phone numbers.
Plan of Correction:	Provider #2 Sworn Statement is complete.
Standard #:	8VAC20-800-170-A
Description:	Based on record review and provider interviews, the provider did not ensue to obtain from each provider each caregiver at the time of hire and each adult household member prior to coming into contact with children a current report of Tuberculosis Screening (TB) form published by the Virginia Department of Health or a form consistent with it documenting the absence of tuberculosis in a communicable form. Evidence: Provider #2 did not have a copy of TB screening available for review. Provider #2 was 232 was observed working on 10-18-2023.
Plan of Correction:	Provider #2 TB screening is scheduled for Monday, October 23rd at 10:30 AM.
