An unannounced monitoring inspection was conducted on on 9/29/2023. Upon arrival at 12:30, one child was present under the care of the Provider. The Licensing Inspector reviewed 1 child record and 4 household/staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.036-A Description: Based on record review, the Licensee failed to update the required Background Checks every 5 years.
EVIDENCE:
The most recent Fingerprint Background Check for Household Member #1 was 9/14/2018 and was not updated within 5 years.
The most recent Fingerprint Background Check for Staff #1 was 8/28/2018 and was not updated within 5 years.Plan of Correction: Background checks will be updated. Documents will be kept in the file.
Standard #: 8VAC20-800-760-A-1 Description: Based on inspection, the Licensee failed to have the required first aid supplies available wherever children are in care.
EVIDENCE:
The first aid supplies did not include triangular bandages and First Aid Manual.Plan of Correction: Provider will purchase missing first aid supplies.
Standard #: 8VAC20-800-830-A Description: Based on documentation review, the Provider failed to ensure emergency evacuation procedures are practiced monthly with all caregivers and children in care during all shifts that children are in care.
