Comments:
An unannounced monitoring inspection was conducted on July 20, 2023, in response to a self-reported incident regarding supervision and transportation that was received by the Office of Child Care Health and Safety on July 12, 2023. The inspectors and other local agencies conducted interviews with caregivers and documentation was reviewed including one child?s record and one caregivers? record. The inspector completed an additional interview on October 13, 2023.

The preponderance of evidence gathered during the investigation supports the self-reported allegation; therefore, the allegation was determined to be valid.

Standard #: 8VAC20-800-1010
Description: Based on interviews, the provider did not ensure that during transportation of children that no child is left unattended inside a vehicle.

Evidence:

1. On July 11, 2023, caregiver #1 transported nine children, ranging in age from three years to nine years, to a movie theater. The caregiver and children arrived at approximately 12:30 pm.
2. After the movie ended at approximately 1:50 pm, caregiver #1 was met in the theater lobby by an unknown person, who had brought child A inside after finding the child unattended and crying inside the vehicle.
3. Caregiver #1 confirmed that child A (three years old) was left unattended inside the van after failing to verify that all children exited the vehicle before entering the theater.

Description: Based on interview, the provider did not ensure during the transportation of children, the driver possessed all required information.

Evidence:

The provider stated that during the fieldtrip on 07/11/2023, caregiver #1, who was driving the vechile, did not have a copy of each child?s emergency contact information as required.

Description: Based on interviews, the provider failed to ensure that the home?s activities and services are conducive to the welfare of children in care.

Evidence:

1. On July 11, 2023, caregiver #1 transported nine children, ranging in age from three years to nine years, to a movie theater. The caregiver and nine children arrived at approximately 12:30 pm.
2. After the movie ended at approximately 1:50 pm, caregiver #1 was met in the theater lobby by an unknown person, who had brought child A (three years old) inside after finding the child alone and crying in the vehicle.
3. Caregiver #1 was unaware child A was missing from the group for the duration of the movie.
