1) 110: The current form approved by the Virginia Department of Health or a physician?s form shall be used to record immunizations and the results of the required physical examination. Each report shall include the date of the physical and dates the immunizations were received and shall be signed by a licensed physician, the physician?s designee or an official of the health department.

An unannounced renewal inspection was conducted from 2:15 - 3:15pm with the provider. There were eight children in care, ranging from three years to six years of age, for a total of 14 points. Children were observed resting/napping during the inspection. Four child records and five provider/household member records were reviewed along with the daily attendance, emergency drill log, menu and required posted information. The provider does not have certification in Medication Administration Training (MAT) so cannot administer medications to children in care. Standard #: 8VAC20-800-60-B Description: (Repeat Violation) Based on review of four child records, it was determined that not all records contained all required information. Evidence: the records for child #1 (date of first attendance: 6/26/2023) and child #3 (date of first attendance: 9/4/2023) did not contain the complete name. address, and phone number for two designated people to contact in case of emergency if the parent cannot be reached or the name and phone number for the child?s physician.
