Comments:

An unannounced monitoring inspection was conducted on 10/23/23. At the time of entrance, 6 children were in care with the provider. The record sample size consisted of 3 children's records and the provider's record and the household member's record. The children in care were observed engaging with the provider, participating in snack and free play. There have been no injuries since the last inspection and no medications are being administered at this time. Areas of noncompliance are noted in the violation narrative and were discussed with the provider during the exit meeting at the conclusion of the inspection.

Description:	Based on record review and interview it was determined the provider did not review with the parent the emergency contact information required in 8VAC20-800-60 B 2 to ensure the information is correct, and obtain the parent's signed acknowledgment of the review. Evidence: 1. The records for child #1(enrolled 8/23/21) and child #2(enrolled 9/14/21) did not contain documentation of parental annual review. 2. Provider confirmed that these records have not been reviewed and updated if needed by the parents.
Plan of Correction:	Provider had parent review and document review at pick up.

Description:	Based on record review and interview it was determined the provider did not ensure that each child's record contained all of the required information. Evidence: 1. The record for child #3(enrolled 2/17/17) did not contain the first page of the child's record. 2. Provider confirmed that the record was not complete.
Plan of Correction:	Provider has requested information from parent.
