An announced initial inspection was conducted today from 9:30am to 10:30am. There are 2 children (7 points) with Provider present today, within the supervision guidelines. Provider and 5 household members records were reviewed and found incomplete. Two enrolled children's files were reviewed. Overall, good recordation system in place. The home was found clean and supplied with ample toys and programming equipment appropriate for the children currently in care. Adequate space determined for the requested capacity of 9 children. Children were observed being dropped off by their guardians, playing freely with blocks and cars, listening to music and being read to. Provider is not MAT training so currently no medications can administered. Emergency procedures, provider contract, First Aid/CPR certification was verified. One crib found on site and meet the CPSC requirements.

Standard #: 8VAC20-800-60-B Description: Based on record review, childrens records did not contain all required information.
Evidence:
1. Child #1s record does not contain verification that the family has been provided with the providers emergency preparedness plan and liability insurance declaration.
2. Child #2s record does not contain an address for one of their emergency contacts, the families medical insurance policy number, and verification that the family has been provided with the providers emergency preparedness plan and liability insurance declaration.Plan of Correction: We will get the information from the families next week.
