Comments:
An unannounced monitoring inspection was conducted on October 25, 2023 between the hours of approximately 10:30 a.m. and 11:05 a.m. There were 9 children present with the Provider and 2 assistants for a total of 18 points within the supervision guidelines. The children were observed playing outside, playing with blocks and completing a worksheet. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Thank you for your cooperation. If you have any questions, please e-mail me at tameika.king@ doe.virginia.gov.

Violations:
Standard #: 8VAC20-800-750-A
Description: REPEAT VIOLATION

Based on observation, the family day home failed to meet all requirements when topical skin products, such as diaper ointment and sunscreen, were used.

Evidence: There were three diaper creams, one sunscreen, one eczema cream and one healing ointment that were not labeled with the child?s name.

Plan of Correction: We already write the names on the creams

Standard #: 8VAC20-800-750-B
Description: REPEAT VIOLATION

Based on observation, the family day home failed to keep topical skin products inaccessible to children.

Evidence:
1. There were three sunscreens, two diaper creams and one eczema cream in a plastic storage drawer, accessible to children.
2. There were two diapers creams and one healing ointment on the diaper changing table, accessible to children.

Plan of Correction: We put all the creams on a lock cabinet

Standard #: 8VAC20-800-750-D
Description: Based on observation, the family day home kept topical skin products beyond the expiration of the product.

Evidence:
1. There was a sunscreen located in a plastic storage drawer that expired in February 2023.
2. There was an eczema cream located in a plastic storage drawer that expired in June 2023.

Plan of Correction: This cream was throwing away

Standard #: 8VAC20-800-690-E
Description: Based on observation, the family day home failed to dispose of diapers in a plastic-lined storage system that is either foot operated or used in such a way that the caregiver?s hand doesn?t touch the exterior surface.

Evidence: The foot-operated trash can used for soiled diapers was inoperable, causing the caregivers to have to lift the lid using their hands.

Plan of Correction: We will buy a new trash can
