Comments:
An unannounced monitoring inspection was conducted on 11/01/2023. At the time of entrance, 11 children were in care with the provider and assistant providers. The record sample size consisted of 5 children's records and the provider's record, the assistants' records, and the household member's record. The children in care were observed engaging with the providers, participating in educational activities, and nap. There have been no injuries since the last inspection and no medications are being administered at this time. Areas of noncompliance are noted in the violation narrative and were discussed with the provider during the exit meeting at the conclusion of the inspection.

Violations:
Standard #: 8VAC20-800-780-B
Description: Based on record review and interview it was determined the provider did not ensure that the parents reviewed and documented the review of the information in their child's file annually.
Evidence:
1. The records for child #1(enrolled 8/17/22), child #2(enrolled 8/15/22), and child #5(enrolled 8/17/22) did not contain documentation of an annual review.
2. Provider confirmed the information had not been reviewed.

Plan of Correction: Provider will review information with parents and document the update.

Standard #: 8VAC20-800-180-A
Description: Based on record review and interview it was determined the provider did not ensure all caregivers had a current Report of Tuberculosis Screening form, in accordance with the requirements in 8VAC20-800-170, every two years from the date of the first screening.
Evidence:
1. The record for assistant #2(hired 1/15/20) did not contain documentation of a current TB screening. The screening in the record was dated 8/4/21.
2. Assistant #2 confirmed that there was not an updated form available during this inspection.

Plan of Correction: Assistant will get an updated TB screening.
