Comments:
An unannounced renewal inspection was conducted today between 9:50am and 11:15am. There were 7 children present with the Provider for a total of 16 points within the supervision guidelines. The children were observed participating in circle time and playing freely. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Violations:
Standard #: 8VAC20-800-750-A
Description: Based on observation, topical skin products were used after they expired.
Evidence: Child #7's topical cream expired 7/2022.

Plan of Correction: I'll give it back to them. I'll start checking the date when parents give me cream.

Standard #: 8VAC20-800-940-A
Description: Based on observation, refrigerator temperatures were not maintained at or below 40 degrees.
Evidence: The thermostat on the refrigerator was 43 degrees during the inspection.

Plan of Correction: I'll drop the temperature and start checking it everyday.

Standard #: 8VAC20-800-960-E
Description: Based on observation, bottles were not dated.
Evidence: Two out of two bottles belonging to child #5 were not dated.

Plan of Correction: I'll start dating the bottles.

Standard #: 8VAC20-800-50-A
Description: Based on record review and interview, the family day home did not keep an accurate written record of children in attendance.
Evidence: Upon a review of the attendance documented during todays inspection and the children present in the home, it was confirmed that the provider had not written the correct children's names on the attendance sheet.

Plan of Correction: I wrote the names down quickly. I'll start writing the names down when the children come in.

Standard #: 8VAC20-800-60-B
Description: Systemic Deficiency
Based on record review, children's record did not contain all required information.
Evidence: Child #2s record does not contain the families medical insurance policy number.

Plan of Correction: I will ask the parents to send the information.

Standard #: 8VAC20-800-180-A
Description: Based on record review, the provider did not obtain a subsequent tuberculosis (TB) screening every two years.
Evidence: The providers most recent TB screening is dated 9/1/2021.

Plan of Correction: I will go this weekend.

Standard #: 8VAC20-800-280
Description: Based on observation, potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents, disinfectants, deodorizers were not stored away in areas inaccessible to children.
Evidence: Several cleaning agents were observed in the cabinet under the sink in the children's bathroom. The cabinet was not locked.

Plan of Correction: I'll move them to the kitchen.

Standard #: 8VAC20-800-430-A
Description: Based on observation, children under two years of age have access to stairs.
Evidence: There is no safety gate at the bottom of the staircase where the family day home program takes place. Child #5 (9 months old) was observed in care during today's inspection.

Plan of Correction: I'll put a gate up.
