Comments:
A renewal inspection was conducted today between 9:25am and 11:25am. There were 8 children present, with 2 caregivers supervising. Children were observed playing with various age-appropriate toys, eating snack, diapering, handwashing and bottle feeding. Positive interactions were observed during this inspection. A total of 4 child records and 2 caregiver records were reviewed. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, nutrition, special care and emergencies. There was no medication on-site at the time of inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 22.1-289.035-A
Description: Based on record review and interview, the provider failed to ensure the criminal history background check was repeated every five (5) years.
Evidence:
1. The record for Caregiver #2 did not contain documentation of a 5 year repeat for a criminal history background check (expired on 8/29/2023).
2. The provider acknowledged the 5 year repeat search had not been completed.

Plan of Correction: I could not schedule an appointment with my iPhone. I have a person who is going to bring their computer and help me get the appointment scheduled for this Saturday.

Standard #: 22.1-289.036-A
Description: Based on record review and interview, the provider failed to ensure the criminal history background check was repeated every five (5) years.
Evidence:
1. The record for Caregiver #1 did not contain documentation of 5 year repeat for criminal history background check (expired on 10/09/23).
2. The provider acknowledged the 5 year repeat had not been completed.

Plan of Correction: I could not use my iPhone to schedule an appointment. I have a person coming today who is going to help me schedule an appointment for this Saturday.

Standard #: 8VAC20-800-760-A-1
Description: Based on observation, the first aid kit did not contain the minimum supplies.
Evidence: The first aid kit did not have more than 1 triangular bandage.

Plan of Correction: I will order more triangular bandages this week.

Standard #: 8VAC20-800-90-A
Description: Based on record review, the provider failed to obtain documentation that the child had been adequately immunized before the child's first day of attendance at the family day home.
Evidence:
1. The date of enrollment for Child #3 was 5/01/23 and the immunization record was dated 05/05/23.

Plan of Correction: I will make sure I get immunizations on the first day.
