Comments:
An unannounced renewal inspection was conducted on 11/8/2023 from 8:20 am-9:35 am. There were 4 children supervised by 2 direct caregivers. The home, 2 caregivers records, 5 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed engaging freely. Areas of non-compliance are identified in the Violation Notice. Please submit your plan of correction within 5 business days for it to be included on the website. Standard #: 8VAC20-800-760-A
Description: REPEAT VIOLATION

Based on observation and interview, the first aid kit did not contain all the required items.

Evidence: The first aid kit is missing adhesive tape and the first aid instruction manual.

Plan of Correction: A new first- aid kit has already been provided w/all needed items and the instruction manual

Standard #: 8VAC20-800-780-B
Description: REPEAT VIOLATION

Based on record review and interview, the provider did not annually review with the parent the emergency contact information to ensure the information was correct and obtain the parent?s signed acknowledgment of the review.

Evidence:
1. Child #3's last annual emergency contact review was on 8/9/2022.
2. Child #4's last annual emergency contact review was on 4/22/2022.

Plan of Correction: The emergency contacts have been updated already.

Standard #: 8VAC20-800-210-A
Description: REPEAT VIOLATION

Based on record review and interview, caregivers did not obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

Evidence:
1. Between September 2022 and September 2023, training documentation showed that Caregivers #1 recieved 9 hours of annual training and Caregiver #2 received 5 hours of annual training.

2. The provider confirmed that the annual training period for the family day home begins in September.

Plan of Correction: The remaining necessary training will be completed by both care-givers.

Standard #: 8VAC20-800-240-A
Description: REPEAT VIOLATION

Based on observation and interview, areas and furnishings of the family day home inside were not maintained in a clean, safe, and operable condition.

Evidence: In the playroom, there were several areas on the wall near the sliding glass door where the paint was chipping.

Plan of Correction: When there is appropriate time and safety the paint will be re-done, The daycare is always cleaned daily + once a week deep clean.
