Comments:
An unannounced monitoring inspection was conducted on 11/13/2023 from 10:40 am to 11:35 am. There were 10 children (28 points) supervised by 2 direct caregivers.

The home, 2 caregivers/ household member?s records, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed engaging freely and eating AM snack.

Areas of non-compliance are identified in the Violation Notice. Please submit your plan of correction within 5 business days for it to be included on the website. Standard #: 8VAC20-800-100-A
Description: Based on record review and interview, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence: There was no documentation of a physical examination completed by or under the direction of a physician prior to or within 30 days of Child #5's first day of attendance. The start date for Child #5 is 10/10/2023.

Plan of Correction: I will ask the Parent to provide the Physical Examination documentation for child #5.

Standard #: 8VAC20-800-70-A
Description: Based on record review, the provider did not provide all the required written information to parents prior to the child's first day of attendance.

Evidence: 4 out of 5 children's records did not contain written information on the family day home's medication policy.

Plan of Correction: I will provide the written information to the parents, and it will be signed by the parent as soon as possible.
