Technical Assistance:
I discussion was had regarding being over ratio. You must have a second person present with the children once you reach 17 points.

Comments:
A monitoring inspection was conducted today between 9:00am and 10:15am. There were 7 children present with 1 caregiver supervising. A second caregiver arrived approximately 10 minutes later. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. The children were observed coloring. A total of 7 child records and 2 caregiver records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-800-780-B
Description: Based on review of children's records, the provider did not annually review with the parent the emergency contact information required to ensure the information is correct, and
obtain the parent s signed acknowledgment of the review.
Evidence
The record for child # 5 does not contain documentation of an annual review with the parents of the emergency contact information.

Plan of Correction: I will have the parents review the emergency contact information and document it has been reviewed.

Standard #: 8VAC20-800-100-A
Description: REPEAT VIOLATION
Based on review of children's records, the provider did not obtain documentation of a
physical examination prior to a child's attendance or within 30 days after the first day of attendance.
Evidence
Children #1, #2, #3 and #4, first day of attendance is over 30 days ago, do not have documentation of a physical examination in their record.

Plan of Correction: I will contact the parents and get a copy of the missing physical form.

Standard #: 8VAC20-800-570-A
Description: Based on observation, the provider did not ensure they were in ratio and ensure they do not exceed 16 points.
Evidence
Upon arrival of the inspector there were 7 children in care for a total of 21 points with only one caregiver.

Plan of Correction: I have an assistant, she is usually here. She just ran upstairs to take the trash out. She came down after I called her to come.
