Comments:
An unannounced monitoring inspection was conducted today between 3:40pm and 5pm. There were 6 children present (in addition to 1 household child) with the Provider for a total of 18 points outside the supervision guidelines. The children were observed finishing an afternoon snack, playing freely with blocks, cars and puzzles, and being picked-up by guardians at the end of the day. A complete inspection of the physical plant, children and staff records, fire drill log, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Standard #: 8VAC20-800-830-C
Description: Based on record review, documentation of emergency evacuation drills was not maintained.
Evidence: There is no fire drill documented for the month of October.

Plan of Correction: I forgot to write it down. I'll make sure I write it down.

Standard #: 8VAC20-800-920
Description: Repeat Violation
Based on observation, menu requirements were not met.
Evidence: There is not food menu posted.

Plan of Correction: My rep told me that she would be here and she brings me menus. I'll have to get a copy from her.

Standard #: 8VAC20-800-50-C
Description: Based on observation, children's information was not kept privileged and confidential.
Evidence: The supplemental from the most recent inspection which clearly states "DO NOT POST" was posted within the inspection notice.

Plan of Correction: Corrected during the inspection.

Standard #: 8VAC20-800-60-B
Description: Repeat Violation
Based on record review, childrens records did not contain all required information.
Evidence: Child #6 record does not contain documentation that the family has received the providers emergency preparedness plan.

Plan of Correction: I'll get the form signed by the parents tomorrow.

Standard #: 8VAC20-800-320-B
Description: Repeat Violation
Based on observation, child-resistant protective covers were not installed on all unused electrical outlets accessible to children under five years of age.
Evidence:
1. Two outlets against child #2's crib did not have protective covers.
2. One outlet next to the nap room door did not have a protective cover.
3. One outlet next to the map table did not have a protective cover.
4. Two outlets next to the couch did not have protective covers.

Plan of Correction: I will have to order more.

Standard #: 8VAC20-800-570-A
Description: Repeat Violation
Based on observation and interview, the provider did not ensure that a caregiver did not exceed 16 points.
Evidence:
The provider was observed working alone with 7 children (18 points); Child #1, 3 years. Child #2, 11 months. Child #3, 3 years. Child #4, 3 years. Child #5, 15 months. Child #6, 17 months, Child #7, 6 years.

Plan of Correction: My assistant will be back tomorrow.
