Technical Assistance:

Discussed with provider: *Door knob covers for bedroom doors in the hallway, wooden pressure gate use to prevent unrestricted access to the kitchen area. *Use of second bathroom to store cleaning products for the daycare bathroom.

Comments:

An announced, Initial inspection was conducted today. There are no children enrolled yet, in the daycare. 5 Caregiver/resident records were reviewed. All were complete. There is one white crib available for use, with a manufacture date of 2020. there are 4 daycare cots available. Emergency supplies, first aid supplies, age- appropriate toys, games and activities, menu, fridge/freezer temperatures, landline and landline phone were all in compliance today. Standard #:	22.1-289.058
Description:	Based on observation and provider statement, there was no carbon monoxide detector installed in the home that was built before 2015. Evidence: No Carbon monoxide detector was installed in the home.
Plan of Correction:	A CO2 monitor will be purchased.
Standard #:	8VAC20-800-800-A
Description:	Based on a review of the written emergency preparedness plan, the plan did not include all required components. Evidence: The written plan available did not include: Assembly points for emergency evacuation, relocation destination address, method to alert provider and children of an emergency, methods of communication with parents.
Plan of Correction:	The model form (template) will be updated to include all required information.

Description:	Based on a review of required information for parents, policies and procedures were not established and in writing. Evidence: Missing policies and procedures include: Holidays, hours of operation, payment schedule, check in and check out procedures, substitutes, termination policy (Information for Parents model form).
Plan of Correction:	The policies and procedures will be finished and put in writing on the model form.

Description:	Based on observation of the crib mattress, the mattress was not covered with a waterproof material. Evidence: The mattress available for the crib, was covered with a soft cloth material, It is not waterproof.
Plan of Correction:	A waterproof covering will be purchased for the crib mattress.
Standard #:	8VAC20-800-690-E
Description:	Based on observation, soiled diapers are not able to be disposed of in a leak-proof foot operated storage system. Evidence: No foot operated, diaper disposal container was available for use.
Plan of Correction:	A foot operated trash can will be purchased.
