Comments:
An unannounced renewal inspection was conducted on October 19 2023 between 11:15am and 12:30pm. There were 2 children in the care of the provider. Children were observe playing, eating a snack, and preparing for lunch time. Staff records, child records, emergency forms, attendance, and the new zoning paperwork was reviewed and inspected. Standard #: 8VAC20-800-70-A
Description: Based on record review, the provider did not ensure that all child records had documentation showing the parents have been notified in writing the parental agreements.

Evidence: Child #1 and child #2 records are missing Medication Administration document, the disclosure of liability document, and the provisions of emergency preparedness.

Plan of Correction: Will get the documents from the parents.
