Comments:
An unannounced monitoring inspection was conducted on Thursday, November 16, 2023, with the provider. The inspection began at 12:10pm and concluded at 1:30pm. There were 9 children present (19 points), with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5child records, 1 staff record, and 1 household member record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-A
Description: (Repeat Violation) Based on record review and interview, the center failed to ensure that all employees shall undergo background checks every five years.

Evidence:
1. The record for caregiver #1 contained a national criminal background check dated 9/28/2018 and a central registry background check dated 8/9/2017. Caregiver #1 should have undergone a criminal background check on or before 9/28/2023 and should have completed a search a central registry on or before 8/9/2022.
2. Caregiver # 1 verified that an updated criminal background check and search a central registry had not been completed.

Description: Based on review of 5 children?s records, the provider failed to ensure that all the required information was on file.
Evidence:
1. The record for child #1 (DOE 9/5/2023), and child #2 (DOE 6/14/2023), did not The Information for Parents form.
2. The provider stated that she thought the forms were completed.

Upon the arrival of the Licensing inspector at 12:10 pm on 11/16/2023, there were nine children in the care of 1 caregiver, totaling 18 points.
2. The provider confirmed that she was the only caregiver to the nine children (18 points).
