22.1 BACKGROUND CHECKS; CARBON MONOXIDE

Comments:
An unannounced monitoring inspection was conducted from 12:00 pm - 1:00 pm. There were two children in care with the provider for a total of six points. The children were observed resting and eating lunch. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records and one provider records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description: Based on review of the provider?s record it was determined that not all background checks were repeated every five years. Evidence: The most recent determination letter based on fingerprinting for the provider is dated 03/09/2018.

Description: Based on observations and interview with the provider it was determined that an attendance record documenting the children in care was not kept daily. Evidence: There was no documentation of today?s daily attendance record at the family day home.

Standard #: 8VAC20-800-240-A
Description: Based on observation of the family day home it was determined that not all areas of the family day home were in clean, safe, and operable condition. Evidence: 1.) An infant seat and play equipment was stacked on a child-size table that posed a potential falling hazard. 2.) There was evidence of what appeared to be cigarette ashes on two place-setting plates on the dinning room table. The dining room had an odor of cigarette smoke. There was one child seated in a high chair next to the dinning room table.

Description: Based on observations it was determined that not all animal excrement was disposed of promptly and the area cleaned after. Evidence: Dried dog excrement was observed on the living room floor. The provider disposed of the excrement after it was pointed out by the Licensing Inspector, the area of the floor was not cleaned.

Description: Based on observations and interviews it was determined that soft bedding was not prohibited around infants. Evidence: A thick fleece blanket was observed in the crib that is utilized for child B (Age: 6 months).
