Discussed in detail reviewing annually the emergency preparedness and response plan.

Comments:
An unannounced monitoring inspection was started on November 15, 2023 from 12:05 PM--1:25 PM. and concluded on November 15, 2023. At the time of entrance, 5 children (age range 5 months to age 4) totaling 12 points were in care with 1 provider. The sample size consisted of 5 children?s records, 1 household member record and 1 provider record. Children were observed preparing for and having lunch and participating in free play.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program

Violations:
Standard #: 22.1-289.036-A
Description: Based on record review and provider interviews, the licensee did not ensure to obtain updated background checks as required every 5 years.

Evidence: Provider #1?s record did not include an updated sworn statement as the most recent was dated 10-11-2018.

Plan of Correction: Signed/dated (11/15/2023) sworn statement for both the provider and household are completed

Standard #: 8VAC20-800-520-C
Description: Based on observation and provider interviews, the provider did not ensure that cribs shall meet the requirements.

Evidence:
The provider did not ensure that the mattresses fit snugly next to the crib so that no more than two fingers can be inserted between the mattress and the crib. The crib mattress used for Child #4 did not fit snugly in the crib.

Plan of Correction: Once additional padding was removed from underneath (11/15/2023) the crib mattress fit properly allowing no more than 1 to 2 fingers for spacing.

Standard #: 8VAC20-800-530-E
Description: Based on observation and provider interviews, the provider did not ensure that no soft bedding of any kind shall be used under or around infants including, but not limited to pillows, quilts, comforter, sheepskins or stuffed toys.

Evidence: Child #4 was sleeping in a crib with a mattress that had a filled blanket under it and both the filled blanket and mattress were wrapped in a sheet. There was also a blanket observed, that provider confirmed was used by Child #4 and is considered soft bedding in a crib.

Plan of Correction: In the presence of the inspector during the inspection, 11/15/2023, the additional padding inserted under the mattress was immediately removed. Later that day the parent was notified that regardless that said child is a relative any additional padding to elevate child while sleeping will require a written note from his doctor.

Standard #: 8VAC20-800-540-A
Description: 8VAC20-800-540. A
Based on observation and provider interviews, the licensee did not ensure infant and toddler equipment such as carrier seats, swings and highchairs shall be used according to the manufacturer?s instructions and when occupied by a child, a safety strap shall be used and securely fastened.

Evidence: Child #4 was observed in bouncer and they were not strapped in with the safety strap.

Plan of Correction: I will ensure the use of safety straps whether the infant seats is in use for 1 to 10 minutes.
