Description: Based on child record review, documentation of additional immunizations for one child under the age of 2 years was not obtained.
Evidence:
Child #1 is under the age of 2 years. The last documented immunizations were dated in January 2023.

Plan of Correction: The parent will provide updated immunizations from the 2 year checkup.
