An unannounced monitoring inspection was conducted today from 10:00am-10:50am with the provider on site. There were 10 children with 4 caregivers. A sample of 2 children's files and 2 caregivers/household member's files were reviewed. The children were engaged in the following: playing with toys, singing in Spanish, and participating in circle time. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. Interactions between the children and caregivers were positive. The physical space, attendance records, first aid kit, radio, flashlight, drill record and the Emergency Preparedness Plan were reviewed. There were no medications on site. Areas of non-compliance are identified in the violation notice.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use caregiver names; list caregivers by positions only.
Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Standard #: 8VAC20-800-180-A Description: Repeat Violation
Based on review of two caregivers records, the provider did not obtain for each caregiver a
current report of tuberculosis screening form every two years.
Evidence:
1. Caregiver #1 and caregiver #2 both had a tuberculosis screening form dated 11/07/2021 and had not been updated.Plan of Correction: I will schedule an appointment for both of us.
