Comments:
An unannounced monitoring inspection was conducted today from 9:00am-10:25am with the provider on site. There were 5 children in care and 3 resident children present (17 points) with 1 caregiver. A sample of 2 children's files and 1 caregiver file were reviewed. The children were engaged in the following: playing with toys. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. Interactions between the children and caregiver were positive. The physical space, attendance records, drill record and the Emergency Preparedness Plan were reviewed. There were no medications on site. Areas of non-compliance are identified in the violation notice.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use caregiver names; list caregivers by positions only.

Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Description: Based on observation, the provider failed to ensure that a caregiver does not exceed 16 points by using the following point system to determine if an additional caregiver is needed.
Evidence:
1. The provider had 17 points during the inspection on 11/20/2023 with 5 children in care and 3 resident children. (Children in care: 10 months-4 points, 11 months -4 points, 14 months.-4 points, 3 years old-2 points, 3 years old-2 points, and Resident Children: 5 years old-1 point, 8 years old, 10 years old).

Plan of Correction: Will hire an additional caregiver as needed.
