Technical Assistance:

Technical Assistance was given and information regarding First Aid was discussed.

Comments:

An unannounced monitoring inspection was conducted on November 17, 2023, from 11am to approximately 12:20m. There were 2 children present totaling 6 points and the provider and household member. The Inspector reviewed two children?s records, to provider?s record and household staff records. Children were observed finishing breakfast and free time in the play area. First Aid kits were observed, and a tour of the house was given. No medication is administered and there are no allergies at this time of children in the center. There is no transportation given. Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented. The violation is listed on the violation notice issued to the center and were reviewed with the provider at the exit interview.

Standard #:	22.1-289.035-A
Description:	Based on record review and interview, it was determined that the center did not ensure that all employees shall undergo a background check every five years. Evidence: 1. In reviewing staff records, both staff records had expired background checks. a. Staff #1?s criminal background checks expired on March 20, 2023; b. Staff #2?s criminal background checks expired on March 29, 2023; 2. The Family Day Home confirmed that the staff member?s criminal background checks were expired as of this year.
Plan of Correction:	The provider responded with the following: The provider will ensure that all staff complete their background check and have the information forwarded to the Department of Education within 10 business days.
