Technical Assistance:
Technical Assistance was given and standards regarding training certificates and hours were discussed.

Comments:
An unannounced monitoring inspection was conducted on November 17, 2023, from 10am to approximately 11am. There were 10 children present totaling 25 points and four providers. The Inspector reviewed five children?s records, the provider?s record and four additional staff records. Children were observed finishing breakfast and free time in the play area. First Aid kits were observed, and a tour of the house was given. No medication is administered and there are no allergies at this time of children in the center. There is no transportation given. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the provider at the exit interview.

Violations:
Standard #: 22.1-289.035-A
Description: Based on record review and interview, it was determined that the center did not ensure that all employees shall undergo a background check every five years.
Evidence:
1. In reviewing staff records, all five staff records had expired background checks.
a. Staff #1?s criminal background checks expired on September 17, 2023;
b. Staff #2?s criminal background checks expired on July 30, 2023;
c. Staff #3?s criminal background checks expired on July 25, 2023;
d. Staff #4?s criminal background checks expired on July 26, 2023;
e. Staff #5?s criminal background checks expired on February 26, 2023.
2. The Family Day Home confirmed that the staff member?s criminal background checks were all expired this year.

Plan of Correction: The provider responded with the following: The provider will ensure that all staff members complete their background check and have the information forwarded to the Department of Education within 10 business days.

Standard #: 8VAC20-800-180-A
Description: Based on record review and interview, it was determined that the provider did not ensure that follow-up Tuberculosis Screenings are obtained every two years from the date of the first screening by staff.
Evidence:
1. The records for Staff #1 and Staff #2 contain tuberculosis screening that are past due for update.
a. The most recent tuberculosis screening for Staff #1 is dated May 15, 2018.
b. The most recent tuberculosis screening for Staff #2 is dated May 15, 2018.
2. The provider acknowledged that a new TB screening is required two years from the date of the last screening. The provider confirmed that documents were not available in the records of Staff #1 and Staff #2 to indicate that up to date tuberculosis screenings had been completed.

Plan of Correction: The provider responded with the following: Staff #1 and Staff #2 will obtain their TB screenings this week and will place the negative screening in their file.
