Comments:
An unannounced renewal inspection was conducted on November 8, 2023, from 10:00am to 11:30am with the provider. There were two children, ages 3 and 4 years old, were in the care of one caregiver for a total of 4 points. The children were observed playing with age-appropriate toys, painting, and playing with Play-Doh. Five children?s records were reviewed. The provider and four household member?s records were reviewed and in compliance. The provider has current certification in CPR and First Aid. The first aid kit, flashlight, and battery-operated radio were observed. The most recent shelter-in-place drill was documented as 10/11/2023. The most recent lockdown drill was documented as 9/20/2023.

Standard #: 8VAC20-800-760-A
Description: Based on a review of the emergency supplies, the provider did not have all of the required supplies. Evidence: The emergency supply kit (first aid kit) was missing 1 triangular bandages; the standard requires two triangular bandages.

Plan of Correction: The provider will obtain an additional triangular bandage for the emergency supply kit,

Standard #: 8VAC20-800-830-A
Description: Based on a review of the emergency practice drill log, the provider did not ensure that emergency evacuation procedures were practiced monthly. Evidence: There was no documentation of emergency evacuation drills being conducted annually.

Plan of Correction: The provider will ensure that emergency evacuation drills will be conducted and documented monthly moving forward.

Standard #: 8VAC20-800-100-A
Description: Based on a review of five children's records, the provider did not obtain documentation of a physical examination with 30 days of the first day of attendance. Evidence: Child #1, enrolled 8/7/2023, did not have documentation of a physical examination in their record.

Plan of Correction: The provider will request the missing documentation from the parents and update the child's record.

Standard #: 8VAC20-800-60-B
Description: Repeat Violation - Based on review of children's records, not all children's record contained required information. Evidence: Child #1, enrolled 8/7/2023, did not have two emergency contacts. Children #2 and #3, enrolled 8/24/2022, did not have an documentation of an annual review.

Plan of Correction: The provider will request the missing documentation from the parents and update the children's records.
