An unannounced monitoring inspection was conducted on November 28, 2023 between the hours of 9:30 am through 10:45am. There were a total of six children (20 points) observed with two caregivers. A total of six children's files and three staff/household member's files were reviewed. The children were engaged in the following: toy play, coloring, singing, and other organized activities. Standard #: 8VAC20-800-60-B Description: Based on record review, children's records did not contain required information.
Evidence:
1. Child #6's record (Start date-10/03/2022) did not have documentation of the review of the child's emergency contact information.Plan of Correction: The provider will have the parent to review the emergency contact information in the record and document the record of any changes.
Standard #: 8VAC20-800-90-C Description: Based on record review, the family day home did not obtain documentation of additional immunizations for a child who is not exempt from the immunization requirements.
Evidence:
1. Child #2 (Start date-04/17/2023) did not have documentation of additional immunizations once every six months. The last immunization was dated 03/17/2023.Plan of Correction: The provider will ask the parents for a copy of the updated immunization record for the child and document the record.
