Technical Assistance:

It is imperative that all background checks be repeated every five years. Staff who return to employment require all new documentation.

Comments:

An unannounced Monitoring inspection was conducted today. A total of 10 children (26pts) with 2 staff were onsite. 1 visitor was also onsite who was not providing child care. The children were observed playing with each other and independently and in small groups in the lower level programming space. Toys and equipment were determined age appropriate for the children in care currently. Observation of all 3 levels of the home was made which included inspection of the cribs used on the lower and upper level of the home. The children transitioned to the middle level of the home were they had washed hands and took their seats at the table and high chairs for lunch. Lunch today was chicken, french fries and fresh banana with milk to drink. Ample amounts of food for second servings if requested. A total of 5 children's files and all staff files were reviewed. One parent observed stopping by during lunch time. The violations determined at the last licensing inspection were reviewed and corrections were verified. Some violations found today. Description:	Based on review of staff records, background checks were not completed every five years. Evidence: Staff #2's fingerprint search result was last completed in July 2018.

Description:	Based on review of staff records, a Sworn Statement was not completed for 1 staff. Evidence: Staff #2's last Sworn Statement was dated in July 2018.

Description:	Based on staff record review, one staff record was incomplete. Description:	Based on review of children's records, 4 out of 5 child records were incomplete. Evidence: 1. Child #1 and #2 was missing the verification of proof of identity. 2. Child #3 and #4 was missing the start date of attendance.

Description:	Based on review of documentation, the rabies vaccination for the Provider's pet has expired. Evidence: The provider's dog's rabies vaccination expired in October 2023.
