Comments:
An unannounced monitoring inspection was conducted today between 10:39am-12:00pm. There were 3 children directly supervised by the provider. The home, 2 caregiver records, 4 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely. The provider was actively engaged with the children during free play. Diapering procedures were also observed. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 8VAC20-800-920
Description: Based on observation, a menu for the current one-week period was not posted.

Evidence:
The menu posted in the Family Day Home was dated 3/13/2023.

Plan of Correction: I'll make a new menu, date it correctly, and post it.

Standard #: 8VAC20-800-100-A
Description: Based on record review, the provider did not obtain documentation of a physical examination within 30 days after the first day of attendance for one child.

Evidence:
1.Child #1 began attending the family day home on 9/8/2020. The record for Child #1 did not contain documentation of a physical exam.

Plan of Correction: I will look for the physical in my records. She is scheduled for a yearly check up soon and I will ask for a copy.

Standard #: 8VAC20-800-110-B
Description: Based on record review, the physical examination and dates immunizations were received were not signed by a licensed physician, the physician's designee, or an official of a local health department.

Evidence:
1. The documentation of a well child visit and immunizations received by the child in the record for Child #4 was not signed.

Plan of Correction: I will ask the parents for a copy of the physical provided to the school.

Standard #: 8VAC20-800-70-B
Description: Repeat violation

Based on record review, the provider did not obtain the parent's written acknowledgement of the receipt of all required information.

Evidence:
1. The notice of liability insurance form for parents in the record for Child #2 did not contain the parent's written acknowledgement of receipt and date received.
2. The notice of liability insurance form for parents in the record for Child #3 was signed by the parent, however the form did not indicate whether or not the provider has obtained liability insurance.
3.The notice of liability insurance form for parents in the record for Child #4 did not indicate whether or not the provider has obtained liability insurance, and did not contain the parent's written acknowledgement of receipt and date received.

Plan of Correction: I will correct the form and ask the parents to sign.

Standard #: 8VAC20-800-680-B
Description: Based on observation the provider did not ensure one child's hands were washed with liquid soap and warm running water after diapering.

Evidence:
The provider was observed changing one child's diaper. Afterwards, the provider cleaned the surface and washed her hands. The provider did not wash the child's hands.

Plan of Correction: I will make sure hands are washed. We usually all go wash our hands together after.
