Comments:

An unannounced renewal inspection was conducted on 11/30/2023. There were 9 non-resident children and 1 resident child, ages 4 months-3 years, totaling 25 points, one substitute caregiver and two assistant caregivers were present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, household members, caregiver training, physical equipment and environment, care of children, preventing the spread of disease, emergencies, and nutrition. A total of 5 children?s records, 4 caregiver records, 2 household member records, and 3 pet records were reviewed. The children were observed in free choice activities and during learning time. The inspector arrived for the inspection at 09:50 am and departed at 11:25 am. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #:	22.1-289.035-B-2
Description:	Based on document review and staff interviews, the center failed to ensure that no person shall be hired for compensated employment prior to the facility obtaining an employment eligibility letter upon the completion of a fingerprint background check as required by the Code of Virginia. Evidence: The record for Caregiver 4 contained a documented hire date of 06/21/2023. The employment eligibility letter was dated 08/10/2023.
Plan of Correction:	Will not hire until the fingerprints come back.

Description:	Based on record review, the provider failed to ensure that assistant caregiver records shall contain all of the elements as required by the standards. Evidence: The record for Caregiver 4 did not contain the following information: verification of age; and documentation of the education and training as required by the standards.

Description:	Based on record review, the provider failed to ensure that each child?s record shall contain all of the elements as required by the standards. Evidence: The record for Child 1 did not contain the following information: name, address and telephone number of the child's physician; name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached (the addresses for each designated person was not provided); information on allergies and intolerance to food, medication, or any other substances and actions to take in an emergency situation; name and policy number of the child?s medical insurance; names of persons other than the custodial parents who are authorized to pick up the child; chronic physical problems, pertinent developmental information, and any special accommodations needed; and first date of attendance.
Plan of Correction:	Will have the parent complete the missing information today.

Description:	Based on record review, the provider failed to obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: 1. The most recent immunization record available for Child 3 (2 year 4 months old) was dated 12/04/2022. Documentation of additional immunizations were not obtained every six months for a child under the age of 2 years. 2. The most recent immunization record available for Child 5 (16 months old) was dated 12/20/2022. Documentation of additional immunizations were not obtained every six months for a child under the age of 2 years.
Plan of Correction:	Have communicated with all parents and will contact them again.

Description:	Based on record review, the provider failed to obtain a current tuberculosis screening from each caregiver at the time of hire. Evidence: The record for Caregiver 4 had a documented hire date of 06/21/2023. The tuberculosis screening in the record for Caregiver 4 was dated 10/12/2023.
Plan of Correction:	Will send for TB screening before hire.
