An unannounced monitoring inspection was conducted on 11/28/2023 from 11:40am to 12:50pm. There were 6 children ages 12 months to age 4 years old (14 points) with the Provider. The programming area, attendance, menus, Provider, Assistant and Household member records, 6 children?s records, emergency drills, emergency supplies, and policies were reviewed. There were no medications on site. The outdoor area was not inspected due to the weather. Children were observed participating in art, dramatic play and lunch. Handwashing procedures were also observed. The Provider had current certification in MAT, CPR and First Aid training. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22.1-289.036-A Description: Based on review of 3 Household member records, the provider did not obtain a sworn disclosure every five years from the date of the last sworn disclosure.
Evidence: The record for Household members #1 and #2 did not contain documentation that a sworn disclosure had been completed every five years from the date of the last sworn disclosure.
