Technical Assistance:
Discussion that CPS request of the central registry background check information can be found through the Department of Education's, website: childcare.virginia.gov. There you will find our on-line portal and directions on how to complete. Training links can also be found at this same web address.

Comments:
This renewal inspection was initiated by licensing staff and concluded on 12/01/2023, between the hours of 10 am to 12 pm. There were 2 children present (6 points), ranging from infant to preschool-age, with 1 caregiver supervising. The children were engaged in the following: art, games, toy play, learning, nurturing, story time, and other organized activities. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 4 children records, 1 staff file, and 5 adult Household Member records were reviewed.

Information gathered during the inspection determined that there were (4) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the home.

Standard #: 8VAC20-800-780-B
Description: Based on review, the annual review for each child's emergency contact information did not have a signed review noted for the past year.

Evidence -
1. On the date of inspection (12/01/2023), the most recent review of the emergency contact information listed for Child #1 was dated - 06/25/2022.
2. This was confirmed on the date of inspection by the Provider.

Plan of Correction: Parent will review and sign.

Standard #: 8VAC20-800-60-B
Description: Based on review, 1 of 4 children files indicated that not all of the required information was contained within each child's record.

Evidence -
1. On the date of inspection (12/01/2023), 1 of 2 emergency contacts was listed for Child #4.
2. The start date for Child #4 was listed as - 10/16/2023.

Plan of Correction: Parent will completed needed information.

Standard #: 8VAC20-800-70-A
Description: Based on review, 4 of 4 children files indicated that not all of the required signed documentation was available on site for each child.

Evidence -
On the date of inspection (12/01/2023), the following documents were not available for review:
1. The Provisions of the Emergency Preparedness and Response Plan for: Child #1, #2, #3, and #4.
2. The Information for Parent form, while completed, it was not signed by the Parent of Child #4.

Plan of Correction: Will complete and place on file.

Standard #: 8VAC20-800-180-A
Description: Based on review and interview, 3 of 5 Household Member files indicated that the Provider did not obtain for each adult household member a current Report of Tuberculosis Screening (TB) form (in accordance with the requirements in 8VAC20-800-170), every two years.

Evidence -
1. On the date of inspection (12/01/2023), the following household members (HM) had outdated TB documentation on file: HM #1 (dated - 11/22/2021), HM #2 (dated - 11/27/21), and HM #4 (dated - 11/22/2021).
2. The Provider stated that an appointment to update this documentation has already been scheduled for later this month.

Plan of Correction: Appointments have been scheduled to update this documentation. When obtained it will be placed on file.
